Purpose: The purpose of these operating procedures is to implement procedures for establishing and terminating access to the UA network and business systems supported by Finance and Operations IT.

Scope: These procedures apply to all Finance and Operations network users. Any exceptions to these operation procedures must be approved by the Office of Information Technology.

Operating Procedures: Ensuring appropriate access to the UA network and Finance and Operations business systems is a critical part of maintaining a secure technology infrastructure. The following procedures should be adhered to when adding/terminating FA employees.

New Employees Supervisors are responsible for promptly notifying IT Support when new employees are added to their workforce. IT is responsible for setting up network access, FA Email access, and appropriate Finance and Operations business systems access.

Terminated Employees (and other Employees to Whom Access Should be Denied)
Supervisors (or other responsible persons) are also responsible for promptly notifying IT Support when employees are terminated (voluntary/involuntary) from their workforce, or when access should otherwise be denied. IT is responsible for disabling network access, FA Email access, and appropriate FO business systems access for the terminated employee.

This form should be used to request denial of access for employees.

A report listing all terminated FO employees will be reviewed weekly by IT Support. If an employee is listed on the report in which a supervisor request for account termination has not been submitted, IT Support will notify the appropriate supervisor (or other responsible persons) via email that action needs to be taken. The supervisor will be given one business day to respond to the email. If no response is received by IT Suport, it will be assumed that FO network access, FA email access, and FO business system access should be disabled.
